Use the same .calendarPopup class in Kronolith's Goto menu, don't use
graphics in the popup, and a few other tweaks/cleanups/improvements.

Guess what - you can't really set the height on a <br />. I think
before I was thinking of padding, not margin; just set a bottom margin
on #menu instead.

Any thoughts on merging this?

- Shuffle menu order a little
- Add show/hide calendar selection to the My Calenders page
- Use CSS styling to show which calendars are selected in the menu dropdown
- Remove menu "Show" button since it doesn't even act as a key focus
- Split Create/Edit calendar forms for clarity

We check remote calendars or all_calendars, so no need to check
display_cals. Also, we want to check if display_cals is locked, not
default_share.

Don't let the <select> wrap onto a new line, bloating the menu.

We need at least one empty option.

Use OPTGROUPs in calendar drop down list.

Reorg kronolith menu a bit (new event is now 2nd like in most apps,
goto and search are related so next to each other), and add a My
Calendars link so users have a clue of how to add new calendars, set
permissions, etc - takes about 4 clicks to get to it through prefs.

Oops, we didn't have a $_with_problem setting. Introduce
HORDE_MENU_MASK constatnts as the number of arguments was getting hard
