Rethink caching and cache the bottleneck - the address parsing - instead of
the UI output.  Since the address parsing is used in both the mailbox and
message screens, we get the benefits of caching in both.  However, this won't
be able to be BC for either mimp or dimp.

Do a smarter job of loading some libraries - we pretty much will always need

Refactor code so Auth_imp:: only deals with authenticating to the IMAP server.
Auth_imp no longer automatically opens a connection to the current mailbox. We
must now manually call IMP_IMAP::changeMbox() anytime we are working with the
IMAP stream and require a connection to a mailbox.  Much cleaner, and should
eliminate any issues with clearing the 'recent' flag incorrectly.

Remove wholly unneeded index updating code in IMP_Mailbox::.  How unneeded?
Well, we rip out 10K of code and the new code works better (there should not
need to be a great amount of bug fixes here either since we barely change the
external API calls) and fixes at least one bug.
Looking back at this, wow - my coding stunk (some would say it still does :) ).
