Lots more documentation work

Rearranged things a bit - now we have appendices which will contain details
on driver configuration, and also package/external program installation.

Comments?  I'll be doing more work to flesh this out this week.

Initial import of the DocBook / XML documentation.  This is far
from ready for primetime, but it's a good start.  Have a look at
the README for more information.
