Avoid double encoded parameters. This fixes double sidebars after logins if
not using cookies. URLs passed in with the 'url' parameter, that contain
ampersands or other characters that get encoded are still broken.

I need to test this on a few other installs, but I think this will
work and is much simpler - testing for the existance of the frameset
and if it's there, modifying the login form target, instead of passing
a GET parameter around. This way Horde logins always reload the full
frameset, which is good from a security standpoint, and also fixes
#814.

Remove redundant menu setting. Use the default value in prefs.php instead.

consistent with our treatment of application permissions elsewhere.

Some define() tweaks; signup.php needs to set AUTH_HANDLER (it takes
care of redirecting itself if signup is turned off).

I obviously spoke way too soon on the frameset/login issues. This
commit gets us closer. Not gonna promise anything this time. :)

index changes for frameset detection.

- index.php and services/portal/menu.php need to take care of auth themselves
- update the logic for whether or not to show problem.php links in login screens
to match the new defaults.
- some style, nuke whitespace, etc.

[cjh] Various $no_auth and $self_contained_auth flags have been standardized
into an AUTH_HANDLER constant, which if defined signals the application
that it should not check permissions upon calling $registry->pushApp(),
as the calling script will handle that itself, or is a system-level cron
job/script/etc.
[cjh] Add an option to $registry->pushApp() to specify whether or not to check
application permissions.

[cjh] The 'allow_guests' setting, and $registry->allowGuests(), have been
removed in favor of Horde_Perms application permissions.

Improve initial_app handling.

If initial_app is set in prefs.php and that app allows guests, do not
require authentication before redirecting to that application.

Fix logic problem with no $url.

Revert the last commit. It broke the url if the menu was disabled and no
framset was used.
If stripping the session id is necessary here, it was done in the wrong
place.

Remove old session id from urls passed at login when not using cookies

Switch us over to portal.php as the default view.

Only maintain one set of menu code. This should now degrade gracefully
for non-DOM browsers.

More cleanup; remove all of the bogus echo "\n"; lines after header()
calls that at one point seemed to be needed for mobile devices, but
all of my emulators work without them now.

- New work at mobile interface to Horde
- Horde-level logins work now (IMP auth won't)
- Lots of bugfixes in Horde_Mobile::
- remove old outdated templates

Show problem reporting link on the login screen if it is a guest
application, and a bunch of other small bits to make that work nicely.

Redirect browser to the frameset only if the user is already authenticated
and escape from the frameset on logout.
This will fix a lot of issues like custom menu items only available after
login (administration) or menu bars in the wrong language and is also
necessary for the forthcoming theme preference.
