[cjh] Call the postauthenticate hook in Auth::setAuth(), and allow the
postauthenticate hook to cause setAuth() to fail. Allows postauthenticate
to fire on any event, including transparent authentication, that could
result in a user being successfully logged in.

I'll merge this in a few days if there are no problems with it.

Add some more logout reasons (session ip change logouts, distinguish between
failed logins for some generic reason and bad username/password logins).
Now, Auth drivers explicitly set the login failure reason when authentication
fails.
Still TODO: More cleanups; making sure the driver generated error messages
get displayed to the user in all situations.

Auth::setAuth() shouldn't be called statically anymore.

Better code reuse, and fix a few typos

A few more Horde::functionCheck() calls, and comment/style cleanup.

Much phpdoc cleanups.  isset()->array_key_exists().  Individual drivers
now use _setAuthError() to better explain why authentication failed.
Reorganized some code.

Added LGPL license information (per CODING_STANDARDS) to the top of all
Horde library files.  My fingers hurt.

The last bunch of superglobals patches.

finish removing the $realm parameter

- add application Auth driver to wrap application-provided Horde auth
- update driver capability arrays

Set the 'transparent' credential to 1, so that we can recognize
transparenly authenticated users.

Add the basic IP auth backend (matches client address against
any in an array of CIDR masks)
